Hernie discale: Signes optimales de la présence et du degré de la hernie discale lombaire

 

Il ressort d’une étude réalisée par Vucetic Svensson1 que seules trois signes physiques sont des valeurs diagnostiques sures pour prédire l’existence d’une hernie discale lombaire et sa gravité son type. Cette étude a été réalisée sur 163 patients opérés pour une hernie discale probable. Les trois signes étaient l’amplitude de mouvements sagittale lombaire, le signe de Lasègue, et le signe croisé de Lasègue. Ils ont constaté que ces tests étaient importants pour déterminer la présence d’une hernie discaleainsi que sa sévérité, mais elle n’est pas exacte pour en déterminer le niveau où se situait  l’hernie discale.

 

Hernie discale - symptômes

Même si les signes neurologiques sont importants pour établir une distinction entre la douleur radiculaire et tronculaire, ils ne sont pas d’une grande utilité dans le diagnostic du type ou du niveau de la hernie discale.2 Un tiers des patients avec une absence du  réflexe du tendon d’Achille avait une hernie discale au-dessus de L5-Sl et de la valeur diagnostique de l’absence du réflexe du tendon d’Achille diminue avec l’âge.3 L’absence d’un réflexe du tendon d’Achille est plus fiable que l’absence du réflexe comme signe de la hernie discale, et sa valeur diagnostique augmente significativement si elle est corroborée par un diagnostic d’imagerie.4 Bien l’absence du réflexe rotulien soit de six à sept fois plus fréquent pour les hernies discales au niveau de L3-L4 comparativement aux autres niveaux, un quart seulement des patients avec ce signe avait une hernie discale au niveau L3-L4.1

 

Le plus fort indicateur du type d’hernie était l’amplitude de mouvements sagittale lombaire. L’amplitude de mouvements lombaire diminuait avec l’augmentation de la sévérité de l’hernie. L’amplitude a été négligeable pour la hernie sous-ligamentaire (renflement généralisé avec la participation racine), et diminue progressivement avec une hernie extrue (ligament commun vertébral postérieur commun vertébral postérieur encore intacte) et séquestré (hernie complet au-delà de la face postérieure du ligament commun vertébral postérieur). Le signe croisé de Lasègue, qui n’avait de valeur que s’il était associée au signe de Lasègue unilatérale positif, a aussi été proportionnelle à la sévérité de l’hernie. Par conséquent, l’utilisation à la fois de l’amplitude de mouvements lombaire et le signe de Lasègue a prédit à 74 pour cent les hernie exclues (hernies séquestrées), et à 68 pour cent les hernies extrues (hernies contenues et protrusion). Dans cette étude, le signe de Lasègue était seulement considéré comme positif si la douleur irradiait dans le pied.
